Miles, There is an issue with MS05-019 which, to make a long story short, will cause the symptoms that you are seeing, i.e. SMTP traffic to other sites halts after several hours or days. Its all dependent on the volume of traffic and configuration of the intermediate network hardware. Its an OS issue so it can potentially affect apps other than Exchange. Please see kb898060 for details and the fix/workaround. Good luck and please let us know how you make out.
