Thanks, Ed. No, nobody disabled and reenabled the account. And, in the
last 2 hours, she got access to her mailbox for a few minutes, then was
locked out again, and now has full access to her mailbox again.

This problem seems to be so flaky, and nobody today is having it, that
since it's working now and I have another 20 fires to fight, I'm gonna
chalk this up to the computer gremlins. 

At least I now know the trick of going to mailbox rights and explicitly
granting them to that user, which I didn't know about before.
