I usually like to just telnet to the box via the port and see if it answers. Do a "telnet webmail.mydomain.com 80" at the command prompt from outside the firewall. You'll either get a 'connect failed' or the screen will go blank. (just type 'exit' to get back to the cmd prompt) Then try the same thing from inside the firewall, if it works there and not from outside something is blocking the HTTP request before it's getting to your web server, like your firewall.
