Have him start Outlook with the /CleanRules switch and then add back the one
rule and see if it works.  There might be something broken under the covers.

Exchange 2003, SP2, Outlook 2003

I have a user who's having issues with his rules. They all of a sudden
stopped working. He deleted them all and just created one and it still
doesn't work on the fly. If he goes into his rules and selects it to 'run
now', it works. All the rules are simple moves, "If email is from userB,
move to folderB". If the rule is active, and an email comes in from userB,
an error pops up that says it tried to run a rule but can't find the folder
even though the folder is there. Doesn't matter if it's cached mode or not.
Any thoughts?
