it's a combination of ntbackup and backing up to disk.  we use backup
exec and LTO3 tape libraries and get up to 3500MB/min.  In contrast we
tried Backup Exec and backup straight to disk (attached to the BE
server) and only got about 1300MB/min.  It's true that several years
ago disk was the fastest media for backups, but the new tape drive
technology pretty much smokes it now.  We do have a dedicated backup
infrastructure just for our exchange servers, including it's own
backup network, but we do close to 9 TB's of backups every night in
our 3 datacenters that hold Exchange servers (nightly fulls).

The question for you is how much are you willing to spend on backups
for Exchange.  We consider Exchange to be the most mission critical
app we have, even above our ERP systems, so we spend quite a bit on
it.  you don't need to spend a fortune to get good backups though, if
you don't have a ton of exchange database data to backup every night
then backing up to disk is just as viable as spending 6 figures on a
large LTO library.  Of course the usual advice applies, make sure you
know your backups are completing correctly, do test restores on a
regular basis, have a good DR plan and test it out every now and then,
etc.
