I just had my mind blown -
User running Outlook 2010, the offline address book wasn't updating.  I moved 
the *.oab files and tried downloading the address book.
It progressed about 5% and stopped on Copying Offline Address Book Template 
File.
After some Google-fu I found the answer in an Exchange forum where the wireless 
mouse driver was causing the problem.
I read the answer with skepticism, but after I had the user remove the wireless 
adapter from the USB port it worked.
I still can't believe it caused the problem.
