If there's no indication of problems with the disk, try checking the transaction logs to see if a large number of logs were generated in a short period of time corresponding to the hang. I've seen instances where 5.5 SP4 servers become unresponsive to clients as a result of someone attempting to send a *huge* attachment (500Mb+) and it bogged down the server. If this does prove to be the case then consider implementing internal message size limits.

Is is possible that a malformed mail message could have caused this? Is there a post SP4 patch that might help? This server has never locked up like this in the last 2 years, and nothing has changed recently.
