You can run AV scanning at the gateway; if you are using FW1 then TrendMicro works well with this but there is no real problem running a reputable AV product on the Exchange box either.
Most on this list will tell you Trend is good as is Sybari's Antigen, and I have personal experience of F-Prot's Command Suite and CA's Innoculan (now Innoculate-IT). Neither of these have ever let me down but as with anything, you have to stay on top of the updates. In an ideal world, I would use one AV product in conjunction with a content filtering product at the firewall and a different AV product on the Exchange server.
