Casey, I have seen several instances of corrupted messages getting stuck in the MTS-IN queue as you describe. Most were messages that had a virus and were partially cleaned at a remote site and them mailed out.
Have a look a Q165505 at the MS Support website. Also, are you sure this particular server is at SP4 as this behavious was a problem that wasn't fully fixed until SP2.
