When you look at the properties of the copy of the message in the "sent items" folder, does it show that the distribution list was indeed cc:d? In that case, they cc:d it. Ambiguous address? "Marketing" distribution group got an email that was supposed to go to Mark Jones, and they didn't pay attention when address resolution came up, and went with the default? (One of the judges with my same first name gets email intended for me once in a while...)
I think the user did it, if it's that way in their sent items box.
