I'd the same problem too which was quite puzzling at first. My account was put on hold, release, on hold again and finally removed from the list !
Anyway, I finally realise why I got kick out from the list while troubleshooting another issue. My internet mail gateway seems to have problem randomly with internet emails coming in from the internet. For some reason (unknown yet) the remote internet gateway terminate the connection while trying to deliver the emails ! Sometimes during the DATA. conversation or just after the receiving the email header. I doubt it is my smtp gateway................
