I had a very similar problem the some (name omitted) LH3 R servers where a RAID drive failed and then when replaced with the identical spare, purchased at the same time and sitting in the spares closet, corrupted the whole array on rebuild. No worries was only a 72 GB partition that took me 15 hours to restore. Support was great but the solution sucked.
