Did he by any chance remove the others users from the "to" and/or "cc" lists before he 
sent it?

I have a user who is using Outlook XP SP2 who went to make a change to add users to a 
meeting but also sent a cancellation notice to other users.  I checked it out and its 
true.  Both messages went at the same time.  He never sent a cancellation notice he 
only added a few users to the meeting and wanted to just send the update to them.  An 
idea's?
