I did everything that was on your list except I use SP3 not 4 for Exch55OWA.

Did you reboot the IIS server? After all this which maybe not totally correct step for step...It should work.. It's pretty straight forward..Assuming you are not doing anything special with the IIS server etc....permissions..so on.. there are likely a bunch more way to set stuff up then above...but when in dough.... KISS it works pretty much out of the box, sound like maybe you skip a step somewhere...
