I've actaully created a new test account to try and repicate this issue. 
The first time I tried to open Outlook with the new account the prompt
appeared.  After hitting cancel about 10 times, Outlook worked fine. 
Then, when I let this new account sit for about 1/2 hour the prompt
reappeared.  I will try deleting this profile to see if it makes any
difference as well as look into RFR.
