When you go to the mailbox permissions for that user, can you see whether any account is still selected as the "associated external account" (preferably the user's own account or SELF)

We have a client that gets the following Event ID 9548 error: "Disabled user /o=[domain]/ou=first administrative group/cn=Recipients/cn=[usename] does not have a master account SID. Please use the Active Directory MMC to set an active account as this user's master account." We've tried http://support.microsoft.com/?id=278966, but that didn't help. Several weeks ago, the user had the same problem, and the only way we could fix it was to kill the user & Exchange account and recreate them. But now the problem has returned, and I'm at a loss as to what's causing it. Exchange 2000 SP4 on Windows 2000 SP4 on a Small Business Server 2000 box.
