That was all summed up in comment 28. When doing the testing for comment 28, I did three different scenarios. Only one of these exhibited the fault. I was hoping that this would help you home in on the problem. I'd have hoped that the ~200kB of log files would have given the deterministic view, but happy to create another summary of it in as much detail as I can. Fundamentally, the problem lies with the hints file not being updated when part of a complex delivery fails. This only happens in an obscure and complicated scenario.
