My video card is an STB Velocity 128 with 4meg. I'm running at 31.5 - 64.3 Hsync and
50-100 vertical with a resolution of 1152x864 and 32 depth. My mouse is a PS/2 MS
IntelliMouse 1.1A emulating 3 buttons.
Here's the pointer section of my XF86Config:

Section "Pointer"
Protocol "IMPS/2"
Device "/dev/mouse"
ZAxisMapping 4 5

#[...]

Emulate3Buttons
Emulate3Timeout 50

Hope this helps. Let me know if there's any other info that would be helpful.
