I must live in a protected neighborhood too, then, because I've never 
heard *anyone* say it (that I can recall), except perhaps when we were 
teenagers.  Others may very well use it, but it doesn't occur with 
nearly the frequency you make it out to, unless amongst a very limited 
group.

To me, it's the equivalent of "Drop dead."  Not a very nice thing to 
say.
