(not sure why you need server, but)

Furry Level

Fanworks Accepted
(the notes part can be for anything/other responses)
And there are two typos, I think- derived and involvement
Answers should be yes or no with any explanations going in the 
box.

Type of Fandom- Multi, Print, Television, Movies, Game

If Multi-Fandom, what Fandoms?

If list, what type of posts are allowed?
