It looks fine to me. But then I'm used to filling out information in that format. It's a matter of reading and checking off what would be applicable. Saves the person from thinking about what info may be missing in their submission. Although white on black may look harsh, the text is crisp and readable. You may want to try a lighter background such as off-white or ivory with black text.
