;-) trouble...moi.... ;-)  We use VSS which granted is not web project
friendly.... normally the way we do this is to have a production say a root
branch, a test branch, a UAT branch and a production branch - the root
branch should always contain all fixes / code which exist on all other
branches but will also contain new fixes / code which has not been
propogated upwards.  Any fixes changes we make to say the production branch
will still have to be coded down to the root branch (and to all others).  We
can of course label (i.e. milestone a build).

Mind you, this way of working was inherited from an old system and may not
necessarily be the best way as we do not have such a granular way of
working.

When I convince the big wigs to go over to FC fully then we will adopt a
more milstone CVS type system.

Production environments should only be running code from a maintenance 
branch (for example, branch p230 for FarCry v2.3) or a specific 
milestone build.  A maintenance branch will have all the latest fixes 
for that version.  A milestone release is just a snapshot in time of a 
specific branch.

The very latest code generally does include all fixes -- where relevant. 
  However, the latest code is untested and although relatively stable it 
shouldn't be relied on for production environments.

Pulling the HEAD of the CVS trunk is just going to get you into lots of 
trouble -- unless of course you are looking for trouble ;)
