Have you tried creating a MyRepository object with an Identify.xml
datastream (rather than serving a static XML file)? Maybe the OAI provider
"expects" a URL for identify that identifies a Fedora object/datastream.
Just a thought...

But my attempt to recreate the Identity.xml file I got from the stock Fedora
included OAI (http://fedora.mobot.org:8180/fedora/oai?verb=Identify) failed
to work.
I don't see any documentation on how I, or fedora, creates this file for
oaiprovider (NOTE: I need to get this one working because the Fedora
included OAI failed some tests on OAIster.org - and doesn't seem
configurable)
