Hi Rick Bilonick, The problem with IRS form(fw9.pdf) is that it has been given Reader-extended rights using Adobe Document Server Reader Extensions 5 / 6 (ADSRE 5/6) , which are termed as UB1 rights. These rights are deprecated and no longer supported in version greater than 9.x for reasons given at http://kb.adobe.com/selfservice/viewContent.do?externalId=kb403499.
