I think your brother is on the right track; I would take the risk and go with the exploratory surgery. Anesthesia options today are more varied than in the past and the vets sound compassionate and competent.
This reminds me of Roylene and the 6 feet of red thread; she become ill rather suddenly and we did tons of tests, x-rays, ultrasound, medication and after 8 days she just continued to get worse and was obviouly in great pain.
I got home on a Friday and found her lying in a pool of her own urine, too sick to stand. I rushed her to the vet and they worked on getting her stabilized (high fever; deyhydrated, etc.) which they did overnight. On Saturday, my vet called and said she was stable and I authorized the exploratory surgery. He found 6+ feet of red thread wound tightly around her intestines; infection had set in and almost severed the intestine. She was a little Hellion as a youngster, but I had never known her to eat anything like that, either. Roylene and Lucy (swallowed a needle) are the reason that nothing like needles, thread, string, etc. can be thrown in the trash and, I actually sew (when need be) on the front porch or in my car.....
Tell your brother to hang in there; sending healing vibes for Jezebel; both Roylene and Lucy came thru just fine!!!
