I've had thumb joint problems too. It got so bad I couldn't hold anything.
Dropped cups of coffee, etc.
My orthopedic surgeron recommended surgery. It's called "anchovie" method.
They took a piece of 
tendon from my arm and made a "fake" joint out of it. Routed all the nerves
and tendons through the tendon
and sewed me back up. I thought I'd made a "big" mistake in having it done.
It took almost 8 mos for the 
thumb to quit hurting when I was using it. Of course, I didn't stop work or
using my machines, so my recovery was probably longer
than had I rested it more.

Today it's great. I'd do my left hand in a heartbeat. I have no more pain,
can lift, knit, spin, etc. Working on the machines
is a snap. I can tease the fibers apart, which I couldn't do before surgery.
