In my opinion, the first step in preparing to have a 10 x 13 format score printed is to select the right vendor. You're going to want to find an output shop who has the equipment, skill, and experience to print exactly what you're seeking.

Depends. Sometimes I have seen the cover is included in the page count, sometimes not. If it is included, I am more familiar with seeing the title and instrumentation on the title page, which is customarily on a right-hand page, and in most printed and bound scores, the music also usually begins on a right-hand page.
2) Comb binding for conductor's scores?
I am not a conductor, but I am disenchanted generally with comb binding, because in my experience the comb has about a 20 year life span. My own opinion is that a conductor's score should be bound more durably, and I'd be inclined to go with some type of wire binding. I'd also make certain that the score and parts are printed on archival quality stock.
3) For printing parts, will "your average" print shop be able to handle printing onto a single sheet, which when folded and opened, will yield two pages? At the same 10 x 13 format?
Don't know about your average shop, but there are shops out there that can handle this. If it were me, I'd start with the phone book, and call a number of shops, looking for a house where someone was a musician, and start there. A bit of time in selecting the printer at the start will save time and headaches later, IMO.
