A problem I've noticed on occasion is that of a forte or a fortissimo marking putting levels past some midi threshhold or something, and resulting in an actual reduction in volume.
Never was sure what if anything could be done about the problem, or what exactly triggered it, and HP wasn't necessarily involved. Thought I'd mention it just in case it helped.
