Well, the customer is always right, right? But why not engrave the first page and show the customer what it looks like on 8.5 x 11"? Then it's his or her decision, not yours.
Also she wants the choral score in octavo size. I have seen lots of recent choral scores printed on 8.5 x 11". What do you think?
I've NEVER used octavo, but it might be because the choral folders she's using only hold that size. Again, give the customer what she wants, although you'll have to futz with paper and maybe cutting down since that is NOT a standard office size, nor a size that copy shops can handle. Make sure she understands that when she makes copies she may be stuck with 8.5 x 11", and it might be better to lay it out for that size to begin with.
