I like this suggestion cause it will at least work when authenticating, unlike all the rest suggested here today :) Just one problem - when entering non-ascii name in CREATE USER we have to put it into quotes, right? (Without this I always get "Token unknown" error). Therefore people who do not need case-sensitive logins (I believe that's most of users) will always have to type logins quoted in "CAPITAL" letters when managing users. Not to say that is too big inconvenience, but I do not understand what is paid for. Well, I understand :) That's to be able to have case-sensitive login (when really needed) and to have same rules for logins and roles.
