The front fenders and inners are off.  There is one nut I can see (and easily 
get to) that I'm pretty sure is the one you are talking about. Are you saying 
that all the other bolts are inside the car?

Jim,

If you don't drill out the outermost bolt of the heater assembly case you
need to remove the fender in order to properly remove the heater assembly
case so that you may replace the heater core.  The heater cores are made
outside the U.S.  DO have the new heater core pressure checked at a radiator
shop before installing it.  It's generally free and good insurance against
having to go through the whole process again.
