I've managed to get round the issue using the following procedure.
Not sure if it will help to narrow down the root of the bug but I will
let you decide.

1. Start Firefox
2. Disable the Firebug add on
3. Firefox asks for restart so I oblige
4. Log on to my application (session created and everything working)
5. Enable the Firebug add on
6. Firefox asks again for a restart and again I oblige
7. The tab displaying our web app is automatically created and works
perfectly.  I am able to test my Ext JS app and there seems to be no
session problem.

Obviously this is no fix but it does show that Firebug and Firefox can
work with PHP sessions, even though we have to go to ridiculous
lengths for it to work.

It's still an amazing tool and I'm sure that these hiccups will
resolved.
