Alternatively, the server may not have the MIME-type set for .flv.
On Apache etc. you tend to be fine, but IIS is a pain - it just won't serve things that you haven't set up a MIME-type for. I had this issue last week; once set up, it's fine. One way to test that - if you try browsing, manually, to the .flv file using your browser and get a 404 error (but you are certain that the file is there) you may be looking at a MIME-type error. You'll need to Google how to set up that MIME-type in IIS - I'm afraid I don't know off the top of my head (I avoid IIS as a general rule).
