I discovered the source of the squelch check-mark problem...
I found that there is a bug in the "Plastic" theme. When I changed the
theme to one of the others in the Configure->UI->General pane, the check
marks work fine.

button does not display the check mark. The AFC button does properly

indicator works as it should.
So, there is just some bug in the display of the check mark. The
squelch function works properly, but I just found my self confused
because I thought the squelch was off because there was no check mark
displayed. It took me a few minutes to figure out that it was
actually squelched, and that is why I was not decoding a signal I
expected to see.
I'll just go back to the color display for now...
