I think it makes a lot of sense to be able to drop a reference into the
wikitext that indicates that an attachment should be rendered inline.

I don't like the full URL syntax because then users are baking the
server name into pages; I would much rather have something like
Topic/Attachment.ext syntax.

One note: how will spaces in attachment names be handled? Users will
definitely try and do that (at least my users will!)

That would be a very common use case I suspect.

I've been working on the attachment feature over the last few months
with a slow but steady pace.  Here is the status of the implementation
for the various modules.

The attachments management page ( Attachments.aspx ) was created and
provides the functionality to add new attachments with summary and
display the current topic's attachments.
The attachments are displayed in a list with the name, author,
modification date and summary.

the url of an attachment has the following format

Currently all the implementation is done only using the file system
content provider.

*Formatting rules (parsing)*
The following formats are properly parsed
/Attachment.ext
Topic/Attachment.ext
Namespace.Topic/Attachment.ext
[Topic]/Attachment.ext
Topic/[Attachment name.ext]
Relabeling is supported
A tooltip with the author, last modification date and summary is
displayed when mouse is over the attachment link

*The Attachments property is available from a TopicInfo object.

Next things I'm going to work on:
How attachments create new topic revisions for newsletters and RSS

Delete an attachment
