Source Navigator (originally by Cygnus, then RedHat, but now being maintained on SourceForge) is well worth a try. Right now, the patched version at http://oimanager.de/sn.htm is best, but there should be an "official" release some time soon.
It has more cross-referencing features than MSVC, but I'm not sure about debugger integration. Available for Windows and Unix.
