[fltk.general] Comments not working for certain articles

I wanted to add some comments to this article:
http://fltk.org/applications/articles.php?L449+I0+T+P3+Q#_USER_COMMENTS
.but when I click on the "Submit Comment" button, I get a white page instead of 
a comment input form.
