A combination of 3dForcer and 3d seed file should do the trick. Apply the 3dForcer and add a Visualizer after it. Then you'll be able to check in the FME Viewer if this part is working. If it is OK but the DGN output is not then at least you'll have a better idea of where the problem lies (in the writing). Check the log file closely for clues.
