Neither JIMI, nor JAI support DICOM out of the box, as far as I know. So the first task is to find an image library or JAI/ImageIO plug-in that supports DICOM. The second would be to write an implementation of the ImageReader interface which can be found in the package org.apache.fop.image.analyser.
