<fo:table-header> is the for table-header, but a table isnt valid with
just a header on its own.
The error message that you got is very much informative. But we won't
get such a message if we run using fop batch file.
How you are executing? Are you using any tools?
I just run FOP from the batch file. I suspect you dont see the error
because you submit XML and XSL file which pass through Xalan first. If
you run the XSLT process separately first then you will be in better
position to debug your stylesheet.
