Thanks for the test file. I was able to reproduce and fix the bug. It is fixed in FOP Trunk: http://svn.apache.org/viewvc?view=rev&rev=517415
If you can't use FOP Trunk you can work around the problem by explicitely setting the height or block-progression-dimension property on the block-container.
