I don't know whether is possible using the AFP include-page-segment
extension, but do check: http://xmlgraphics.apache.org/fop/0.95/extensions.html#external-document
Maybe (but I'm not at all sure), it is possible to combine both
extensions. If not, then external-document should still be enough to
include the PDF.
