I had so many issues getting external graphics to work I gave up for now. If we actually need to print a picture I'll have to look at external-graphic again. I print all my pictures right now with SVG. Have you tried SVG? It seems that would be the simplest option if all you need is a simple solid color line.

They all print fine. However, near the bottom of the form, I need to print a light-gray bar from left to right prior to the footer. I tried to do it by coloring a line with a height of one inch. When that did not work, I added the "external-graphic". The graphic works BUT I cannot get the image to be the proper width and height.
