Where is the switch to turn this type of track placement on and off?

When printing to the new Acrobat 5.0 it is now necessary to change the
Conversion Settings from "ebook" to "print" each time a schematic or PCB plot is
done or no plot will be generated. The default settings for the HP1120 print
quality always override the settings saved in Protel but that is probably not a
Protel problem.

No, I don't think so, at least not with the CAM manager. This would require
a better explanation. If you want to re-use a CAM setup with a new file,
just copy it and, if needed, rename it.
