Another reason is that I see absolutely no benefit to having active hyperlinks in the headers and footers of the PDF I produce that take the reader to some strange and non-useful location like the title page. One set of templates I once had to work with used cross-references in the headers and footers, and I absolutely hated it. It's probably possible to make x-refs in headers and footers behave better with some FrameScript scripting, but I just don't see the point.
