And just to be clear, after you apply the custom page for the start of the 
chapter, you do save the file before you update the book, right?
If you want to use the Apply Master Pages feature, you can edit the map Linda 
mentions so there is an entry for the first page of the file, triggered by your 
chapter title format as that should not be used elsewhere in the file.

Is it possible you have a Master Page map applied on your reference page?  To 
check, go to View > Reference Pages. Scroll down until you find the 
UnstructMasterPageMaps.  If this is defined for left and right master pages, 
and you choose to Apply Master Pages when you generate your book, the master 
pages are reapplied. For information about what the table means, search for 
"Assigning master pages" in the FM online help.
