Your problem may well have been using a safelight at all with Tri-X:

Note on page 1:
“No safelight is recommended until after the stop bath. Unprocessed films must 
be handled in total darkness.”

While Tri-X is B&W, it’s also panchromatic, which means there aren’t safe 
wavelengths of light to use while processing.

My processing seems to be sound! While my dark room situation is less than 
ideal (a shed in my backyard with light leaks in the roof), I turned off my 
safe light and got a clear image after developing for about 20 seconds.
