The solution depends on whether he is actually using a Virtual Machine or not, 
and if so which VM.  Some VMs virtualize gamepads/joysticks and some don't.

This would only make sense if he has an old analog/gameport joystick that he is 
particularly attached to for some reason and wants to keep using it.  
Especially on EBay, the cost of a used USB gamepad/joystick is probably about 
the same as a USB-to-GamePort adapter.  You can also get new USB 
gamepads/joysticks without spending a lot of money (depending on how 
"sophisticated" you want them to be).

If you're running DOS inside a Virtual Machine of some sort, it SHOULD be able 
to virtualize a USB gamepad/joystick as a regular analog/gameport joystick 
inside the Virtual Machine.  You may need to play around with some of the VM 
settings to get it to work, or may need to switch to a different VM if the one 
you're using doesn't support joysticks.

If you're running DOS on real hardware, there is only one solution available 
but it does not work with all hardware and can be daunting to set up (USB is 
EXTREMELY complicated).  You will need to use my USB drivers (available at 
http://bretjohnson.us).  AFAIK, those are the only drivers available for native 
DOS that support USB joysticks/gamepads.
