That seems like a replication conflict. Consult the following guide to solve

Just a side question, how did you end up with such entry? Did you happen to 
upgrade
multiple IPA masters at once?

I fear I was doing too few things at the same time: adding / removing replicas and at around the same time upgrading ipa*. Everything last evening.
