That really sounds like a DNS problem. You probably ave hostname_lookups
turned on, and don't have DNS records for some of the IP addresses
that the NAS is sending.

That won't help.
Turn hostname_lookups off in radiusd.conf. If that isn't the
problem, then it's something *really* weird.
